YOU ARE HEREBY SUMMONED and required to serve upon the Petitioner an answer to the complaint which is herewith served upon you, within 20 days, exclusive of the day of service, after service of this summons upon you, if served within the State of Arizona, and within 30 days, exclusive of the day of service, if without the State of Arizona.

YOU ARE HEREBY NOTIFIED that in case of your failure to appear and defend, judgment by default will be taken against you for the relief demanded in the petition.

YOU ARE CAUTIONED that in order to appear and defend, you must file a proper response in writing with the Clerk of the Court, along with the necessary filing fee, within the time required. You are required to serve a copy of any response to your spouse.

A copy of the pleadings being served may be obtained by requesting a copy from the Petitioner at the above address and or phone number or by request to: Clerk of the Superior Court, 110 West Congress, Tucson, AZ 85701. If request is made to the

Clerk of the Court, the case number must accompany that request.
